Health Services
The school nurse helps to promote and maintain the health of all our students to enable them to function optimally as students and future citizens. The nurse conducts New York State mandated vision and hearing screenings, as well as a scoliosis screening. A doctor’s written note with his results is required by the Health Office if you prefer to have your child’s scoliosis screening done by your own doctor.
The nurse should always be informed of any special medical conditions that a child has - either permanent or temporary. She will help facilitate an easy transition from home to school for the student as well as the parent.

ACCIDENTS
When accidents occur during the school day, children are required to report them to their classroom teacher, teacher aides and/or monitors at lunch and recess. The school nurse will assess the situation. If the situation warrants further attention, the school nurse will contact the parent, teacher, school office, and/or the Principal or Assistant Principal.
MEDICATION
Any medication given in school requires the written consent of both the parent and the doctor. This is required by New York State Law and the Somers Central School District and there is a specific form available in the Health Office. A doctor can also write the prescription on his own form with his letterhead and the parent can then sign and date that form. All medication must be brought to school by the parent or designated adult in the original prescription vial or in the over-the-counter labeled containers.

IMMUNIZATIONS
The following vaccines are required for a child to enter school in 3rd, 4th or 5th grade:
Diphteria and Tetanus toxoid-containing vaccine and Pertussis (DTaP/DTP/Tdap) - 5 doses or 4 doses (if the 4th dose was received at 4 years of age or older), or 3 doses (if aged 7 years or older and the series was started at 1 year of age or older)
Polio (IPV/OPV) - 4 doses (or 3 doses if the 3rd dose was received at 4 years of age or older)
Measles, Mumps and Rubella (MMR) - 2 doses (The first dose of MMR vaccine must be received on r after the first birthday. The second dose must have been received at lest 28 days (4 weeks) after the first dose to be considered valid.)
Hepatitis B - 3 doses or 2 doses of adult hepatitis B vaccine (Recombinvax) for children who received the doses at least 4 months apart between the ages of 11 through 15 years of age
Varicella (Chickenpox) - 2 doses
ABSENCE EXCUSES
Regular attendance is not only a key to academic success but it is required by New York State law. When your child is absent, please email

Buerle, please call between 9:00 a.m. and 11:30 a.m. and from 12:30 p.m. to 3:15 p.m. The private number for the Health Office are 914-277-3952.
WHEN A CHILD RETURNS TO SCHOOL
Children should be fever-free or symptom-free, at least 24 hours before returning to school. When your child returns to school, he/she must return with a note explaining
the absence. Please direct your child to bring the note to his/her teacher. The following reasons for student absences from school are recognized as valid:

EXCLUSION/RETURN TO PHYSICAL EDUCATION
Extended excuse from Physical Education classes will ONLY be granted with a physician's note. In other situations, students will be allowed to have limited activity for one physical education class with a note from the parent.
